چکیده
A 5-month-old male infant was admitted with abdominal distension of three weeks duration. The onset was accompanied with few episodes of nonbilious vomiting and sudden appearance of bilateral scrotal swellings. The baby had been passing acholic mucoid stools since two weeks. Physical examination revealed a pale, malnourished, anicteric infant having massive ascites and bilateral hydroceles. Abdominal paracentesis revealed free flow of bile.

Biliary ascites in children due to perforation of bile duct is a rare entity. The exact pathogenesis is not known but there are proposed mechanisms including congenital weakness of ductal wall, pancreaticobiliary malunion, tuberculosis, necrotizing enterocolitis and rupture of choledochal cyst. Presentation may be acute or sub-acute. Progressive insidious course is the common presentation in ch...
متن کاملBile duct perforation in children: is it truly spontaneous?
INTRODUCTION Bile duct perforation (BDP) with resultant biliary ascites in children is a rare clinical condition. The aetiopathogenesis is still an enigma, with increasing evidence suggesting anomalous union of pancreaticobiliary ductal (AUPBD) system as the prime causative factor. CLINICAL PICTURE We report 2 cases of spontaneous perforation of the bile duct confirmed on histopathological ex...

متن کاملSpontaneous biliary perforation in infancy and childhood: clues to diagnosis.
Spontaneous perforation of extrahepatic bile duct is rare. The cause is idiopathic once trauma and choledochal cyst are ruled out. The condition presents a diagnostic dilemma. Preoperative recognition is necessary as early surgical intervention gives excellent prognosis. We report clinical observations made in three cases with acute presentations. Diagnosis is to be suspected by the presence of...
